  • A QUIPS analysis of related factors of poor outcome in children with convulsive status epilepticus

    ObjectiveUsing Quality in prognosis studies (QUIPS) analysis, this paper systematically reviewed the factors influencing the poor outcome of children with convulsive status epilepticus (CSE).MethodsTo longitudinal cohort studies on the prognostic evaluation of CSE mortality and mobidity in children.The retrieval time was from January 2008 to November 2019, and three system reviewers PUBMED, EMBASE, COCHRANE and other databases were used to search for literatures related carried out literature extraction and quality evaluation. According to the QUIPS analysis method, the included literatures were scored, the quality grade was divided, and the analysis variables of medium/high quality literatures with statistical significance were selected to draw a conclusion.ResultsQUIPS analysis was used to assess the literature quality, 17 medium/high quality literatures were included, and the factors with statistical significance (P<0.05) mentioned at least twice or more in≥2 medium/high quality literatures were selected, which were considered as important risk factors affecting prognosis.These factors include: etiology, age, duration of convulsion, refractory CSE, neuroimaging abnormalities.ConclusionFive risk factors indicating poor outcome of CSE in children were summarized. Due to the heterogeneity of various literature studies, Meta-analysis has not been completed, so it has certain limitations.

  • A Meta-analysis of effectiveness and safety of steroids combined with antiepileptic drugs in the treatment of children with epileptic electrical status during sleep

    ObjectiveTo systematically evaluate the efficacy of steroids combined with antiepileptic drugs compared with alone antiepileptic drugs in the treatment of children with epileptic electrical status during sleep to provide evidence-based medical basis for its treatment.MethodsElectronic searches were maded in PubMed, Embase database, Cochrane Library, CNKI, Wanfang and the Chinese biomedical literature database for the literature about steroids combined with antiepileptic drugs compared with alone antiepileptic drugs in the treatment, and retrieval time is from January 1990 to October 2020. Two evaluators independently screened literature, extracted data, evaluated literature quality and risk of bias and checked each other. Meta analysis was performed by stata16.0 software.ResultsA total of 679 children with ESES were included in 10 studies, including 9 randomized controlled trials and 1 retrospective cohort study. Meta analysis results showed that there were statistically significant differences in clinical improvement rate [RR=1.31, 95%CI (1.21, 1.42), P<0.01], electroencephalogram discharge improvement rate [RR=1.35, 95%CI (1.25, 1.46), P<0.01] and cognitive intelligence score [SMD=1.19, 95%CI (0.80, 1.57), P<0.01] between steroids combined with antiepileptic drugs group and alone antiepileptic drugs group after 6 months follow-up. The incidence of adverse reactions in steroids combined with antiepileptic drugs group was higher than that in alone antiepileptic drugs group, and the difference was statistically significant [RR=4.13, 95%CI (1.06, 16.13), P<0.01]. All adverse reactions improved or disappeared after drug withdrawal.ConclusionCompared with alone antiepileptic drugs, steroids combined with antiepileptic drugs group has advantages in controlling epileptic seizures, improving electroencephalogram abnormalities and improving cognitive ability, and it is relatively safe.

  • Association of body mass index and colorectal cancer mortality: a meta-analysis

    Objective To explore relationship between body mass index and all-cause mortality or cancer-specific mortality of colorectal cancer. Methods The published articles relevant body mass index and colorectal cancer mortality were retrieved according to the inclusion and exclusion criteria from PubMed, Elsevier-Science Direct, and Web of Science databases. The meta-analysis was performed with RevMan 5.3 software. Results A total of 14 articles were eligible for the meta-analysis, involved 53 804 patients (colorectal cancer patients with underweight 1 853 cases, colorectal cancer patients with overweight 9 088 cases, colorectal cancer patients with obesity 4 463 cases). The results of meta-analysis showed that the colorectal cancer patients with obesity and underweight had the higher all-cause mortalities 〔RR=1.11, 95% CI (1.06, 1.16), P<0.000 01; RR=1.34, 95% CI (1.11, 1.61), P=0.002〕 and colorectal cancer-specific mortalities 〔RR=1.15, 95% CI (1.05, 1.24), P=0.001; RR=1.33, 95% CI (1.09, 1.62), P=0.005〕 as compared with the colorectal cancer patients with normal weight. The all-cause mortality and colorectal cancer-specific mortality had no significant differences between the colorectal cancer patients with overweight and the colorectal cancer patients with normal weight 〔RR=0.96, 95% CI (0.89, 1.04), P=0.31; RR=1.00, 95% CI (0.90, 1.12), P=0.98〕 . Conclusion Colorectal cancer patients with obesity and underweight might have a higher mortality rate.

  • Clinical effect of ultrasound-guided anterior platelet-rich plasma injection on the treatment of rotator cuff injury

    ObjectiveTo investigate the effect of ultrasound-guided anterior injection of platelet-rich plasma (PRP) combined with routine physiotherapy on pain and functional improvement in patients with rotator cuff injury.MethodsThe patients with rotator cuff injury treated in the Department of Rehabilitation Medicine, the Affiliated Hospital of Southwest Medical University from August 2017 to June 2018 were randomly divided into control group and PRP group. The control group was treated with routine physiotherapy for 4 weeks, and the PRP group was treated with PRP injection guided by musculoskeletal ultrasound (once a week, two injections) combined with routine physiotherapy for 4 weeks. The Shoulder Pain and Disability Index (SPADI) and the European Shoulder Association Constant-Murley Score (CMS) were used to evaluate the changes of pain and function before intervention, 12 weeks after intervention and 24 weeks after intervention.ResultsFinally, 65 patients were included, with 33 in the control group and 32 in the PRP group. Compared with the control group, there were significant differences in the CMS and SAPDI scores between the PRP group and the control group at 12 and 24 weeks after intervention (P<0.05), except before intervention (P>0.05). Twelve and 24 weeks after intervention, the CMS scores of the two groups were higher than those before intervention, while the SPADI scores were lower than those before intervention (P<0.05). The results of repeated measures analysis of variances showed that there was no statistical significance in group effect of CMS or SAPDI scores (F=2.753, P=0.102; F=2.724, P=0.104), but the time effects of CMS and SAPDI scores were statistically significant (F=251.002, P<0.001; F= 1 846.753, P<0.001), and there was interaction between group and time (F=4.931, P=0.020; F=36.405, P<0.001).ConclusionAnterior injection of PRP guided by musculoskeletal ultrasound combined with conventional physiotherapy is more effective than conventional physiotherapy in relieving shoulder pain and improving shoulder joint function.

  • Simulation study on quantitative data in series of N-of-1 trials based on mixed-effect model

    ObjectiveA simulation study was used to generate the multivariate normal distribution data with a residual effect based on series of N-of-1 trials. The statistical performance of paired t-test, mixed effect model and Bayesian mixed effect model were compared.MethodsThree-cycles N-of-1 trials were set, and the participants were randomly assigned to 2 different treatments in each cycle. The simulation study included the following procedures: producing six-dimensional normal distribution data, randomly allocating intervention methods and patients, adding residual effects, constructing and evaluating 3 models, and setting the parameters. The sample sizes were set as 3, 5, 8 and 10, and the correlation coefficients among different times were set as 0.0, 0.5 and 0.8. Different proportions of residual effects for the 2 groups were set. Type I error, power, mean error (ME), and mean square error (MSE) were used to compare the 3 models.ResultsWhen there was no residual effect in the 2 groups, type I errors of 3 models were approximately 0.05, and their MEs were approximately 0. Paired t-test had the highest power and the lowest MSE. When the residual effect existed in the 2 groups, the type I error of paired t-test increased, and its estimated value deviated from the true value (ME≠0). Type I errors of the mixed effect model and Bayesian mixed-effect model were approximately 0.05, and they had the same power. The estimated values of the two models were close to the true value (ME was approximately 0).ConclusionsWhen there is no residual effect (0% vs. 0%), paired t-test is suitable for data analysis of N-of-1 trials. When there is a residual effect, the mixed effect model and Bayesian mixed-effect model are suitable for data analysis of N-of-1 trials.

  • The application of noninvasive local anesthesia for venipuncture in children: a meta-analysis of randomized controlled trials

    ObjectiveTo systematically evaluate the efficacy and safety of local anesthesia for venipuncture in children, and to provide evidence for related nursing practice.MethodsWeb of Science, PubMed, Cochrane Library, Embase, China National Knowledge Infrastructure, Wanfang Data, Chinese Biomedical Database and VIP databases were searched for randomized controlled trials (RCTs) about the application of local anesthesia in venipuncture in children till June 25th, 2021. Two reviewers independently reviewed the literature, extracted data, and assessed the risk of bias of included RCTs. RevMan 5.3 software was used for meta-analysis.ResultsA total of 19 RCTs were included, comprising 2 566 patients. All of them were high-quality English articles included in SCI or Medline. The results of meta-analysis showed that: the painless rate [odds ratio (OR)=3.80, 95% confidence interval (CI) (1.88, 7.66), P=0.000 2] and satisfaction rate of venipuncture [OR=2.12, 95%CI (1.27, 3.54), P=0.004] in the local anesthesia group were higher than those in the non-anesthesia group, and the pain score [mean difference=−0.62, 95%CI (−0.77, −0.48), P<0.000 01] in the local anesthesia group was lower than that in the non-anesthesia group. There was no statistically significant difference between the two groups in the success rate of the first puncture [OR=1.14, 95%CI (0.77, 1.68), P=0.52], the incidence of transient skin reactions [OR=1.15, 95%CI (0.67, 1.95), P=0.62], the incidence of paleness [OR=1.11, 95%CI (0.57, 2.15), P=0.76], or the incidence of edema at the puncture site [OR=0.64, 95%CI (0.21, 1.96), P=0.44].ConclusionsLocal anesthesia can effectively reduce pain and improve the satisfaction of children with venipuncture, and has good clinical safety. It can be used by nursing staff in clinical practice.

  • Study on the reporting checklist of technical specifications for traditional Chinese medicine

    Technology in traditional Chinese medicine (TCM), which has a long history, provides excellent traditional culture and valuable spiritual wealth in China. TCM standardization technology will provide an important basis for guiding and evaluating the professional level and service quality of technicians in TCM. And it is also conducive to the sustainable development of TCM technology. However, at present, there is no relevant research on the development process of technical specifications for TCM. So, the urgent need is how to develop standardized technical specifications, and apply them to patients in practice. Therefore, this study first convened relevant experts, including TCM clinical experts, epidemiologists, methodology experts, etc., to form the core expert group, implementation group and quality assurance task group. According to the personnel functions, they are divided into technical specification steering committee, consensus expert group, secretariat group, external review group, evidence evaluation group, etc. Then, the initial entries are collected by systematically searching the current TCM technical guidelines, consensus and specifications and referring to the existing technical specifications. Finally, expert opinions were collected based on the Delphi survey, and the final reporting checklist of technical specifications for TCM was formed after consensus discussion. The study can provide evidence-based methodological guidance for the development of TCM technical specifications, and promote the standardization and internationalization of TCM technical specifications.
